I clear equally that the player is suitable for the sport and present a medical certificate under my responsibility before the Program.

I inform to WOSPAC that the player has medical and repatriation insurance, and I am providing the information from the medical centers in Barcelona covered by the insurance company where they will attend the player if they need any medical service. In case of not having medical and repatriation insurance, I authorize WOSPAC to get medical and repatriation insurance the player valid during their stay in the Barcelona Program for a cost of 200€ per month.

By signing this document, I am responsible for any injury or damage that may occur to my child or ward throughout the course of the Program. Similarly, the full responsibility of all these facts isn’t for WOSPAC and other participants in the Program. I agree to appear in court assuming all my responsibilities in this present document.

I give WOSPAC the full permission to act on behalf of the parent / legal guardian in any situation during any activity, legal, medical or otherwise. In case of an accident that requires urgent medical intervention, I authorize WOSPAC to make the removal of the player to a hospital for medical service.

I accept and authorize WOSPAC to move my the player named offsite for developing alternative activities and/or leisure.

IMPORTANT:

AUTHORIZATION FOR NIGHT OUT ON BANK HOLIDAYS

AUTHORIZE that the player may leave the residence without supervision by WOSPAC staff the night before a bank holiday, knowing that the next day the athlete does not have training or match. The player may leave the residence alone, knowing that this may represent a risk.

GENERAL TERMS AND CONDITIONS

• Appropriate behavior: The athlete is required to show proper conduct in all aspects of his/her stay. Failure to do so will be grounds for punishment or dismissal from the program.

• Communication for a bad behavior: Any action considered a mild or serious misdemeanour shall be communicated to the athletes and their families. If the improper action or conduct continues, the parents or guardians shall once again be informed, and WOSPAC will reserve the right to decide if the athlete continues or not with the program.

• Misconduct: WOSPAC is not responsible for any illegal actions performed by athletes outside the halls of residences, shared flats or host families. And if the child is under the age of 18, then the parents of the child must be charged with any fines imposed by the police, the athlete will also have to pay any indirect expenses arising from the staff and time used in helping him to rectify the actions.

• Legal Aid: If any athlete need legal help to remedy his/her actions in the Courts of this country, WOSPAC shall make available to his/her the necessary staff. In any case, parents or guardians must pay for the expenses resulting from that procedure.

• Prohibition: It is forbidden to steal, break or vandalise anything (or any uncivil act similar to this, and also to smoke, drink or consume any narcotic substances. In the event that any extraordinary event occurs, advise the tutor that is working as soon as possible. This actions are a serious breach. The athlete who does this kind of actions could be penalized with the expulsion of the program.

• Pets: Animals of any kind are not permitted in the residences, shared apartments or host families.

• Cohabitation: The stay at the halls of residence, shared flat or host family requires proper conduct and compliance with the established rules as well as fulfilment of the tasks assigned for the sake of harmonious coexistence.

• Personal belongings: Athletes are responsible for their personal belongings, and WOSPAC is not responsible to compensate for the loss of these. In the event of any incident, WOSPAC will help athletes to carry out the necessary formalities to retrieve their belongings.

• Medicines: All of the medications that the residents take must have been prescribed by a doctor and the athletes must only consume the quantities stipulated by the doctor.

• Gym:
– People under 18 are not allowed to access the gym.
– For reasons of hygiene, all members must use a towel.
– Footwear must be worn at all times.
– Food may not be consumed in the gym.
– Equipment (weights, training bands etc.) must be returned to their correct place after use.
– You must to respect and follow the indications of the gym’s staff.

• Swimming Pool:
– Children under 18 years of age must be always accompanied by a tutor.
– The use of a swimming cap and swimsuit / swimming shorts is obligatory.
– Sandals must be worn in the area around the swimming pool.
– You may not wear street clothes in the area around the pool.
– Food and drink may not be consumed near the pool.
– You must have a shower before using the pool.
– The pool is used s exclusively for swimming. Anyone can play with balls inside them.

• Spa / Sauna / Steam bath:
– Spa, sauna and Steam bath use restricted to people above 18.
– We recommend you spend no longer than ten minutes in the hydromassage area.

SERVICE PROVISION CONTRACT

ACCEPTANCE OF TERMS AND CONDITIONS:

Reservations and deposits:

Customer Cancellation and Refund Policy:

If the cancellation is made 15 days in advance, at the beginning of the Coexistence, the 50% Refund will be made.
If the cancellation is made less than 15 days in advance, at the beginning of the Coexistence, NO Refund will be made.

Cancellations are effective on the date WOSPAC receives written notice. No refunds will be made for players who voluntarily cancel the WOSPAC program for any reason, nor will any refund be made for a player who is asked to leave the Cohabitations for the use of drugs or alcohol, for the use of any material or object considered as a weapon, for not notifying a severe medical condition or for disobeying the rules of the Coexistence (decisions that will be made at the sole discretion of WOSPAC).

Cancellation or substitution of the operator:

We can cancel any WOSPAC activity at any time before its start, including the case in which the number of participants signed up for a certain activity is sufficient to put it into operation. On the other hand, we will use commercially reasonable efforts to maintain the itinerary as published; however, the final itinerary may vary due to factors beyond our control. We may unilaterally substitute services or goods of similar quality for any of the services or goods indicated in the itinerary. If an activity is cancelled, our liability is limited solely to a full refund of your deposits. If for reasons beyond our control an activity is interrupted or cancelled, we will refund in good faith a part of the price of the activity, based on the actual costs incurred compared to the full activity contracted.

Accommodation:

The accommodation during the Coexistences will be in double or group rooms, with access to a shared bathroom.

Participant’s health certification; responsibility to inform the operator of a disability or pregnancy:

By signing below and/or entering the price of the Cohabitations, you declare that you do not have any condition or disability that could create a danger for you or other people who participate in the Cohabitations, and that you understand that the standards of health care, facilities and services abroad may be different or even insufficient for the treatment of certain health conditions.

You agree to inform us by certified mail or by email sent to info@wospac.com (with acknowledgment of receipt from us) at the time you present the payment of the Coexistence fee (or at any time after , when such a condition is realized) of any physical or mental illness, disability, ongoing pregnancy or any other condition, which may require medical attention or special accommodation during the Coexistence. No guarantee is offered that we can accommodate said condition during the Cohabitations, and we are not responsible or guarantee that we can make special adaptations or access for the disabled in the Cohabitations.

Bad behavior of the participants:

Alcohol, drugs and tobacco are strictly prohibited during the Convivencias. In the same way, any object that can be considered or used as a weapon is prohibited during the Coexistences. Participants must respect other participants and staff, and follow the daily planning of the activity and the established rules. Any serious misconduct will be subject to immediate expulsion. The parent or guardian will be notified immediately and they will need to purchase the return ticket.

Statement for athletes:

You acknowledge that engaging in sports activities is potentially dangerous. Customer agrees that it is subject to such risks and that it voluntarily assumes all risks associated with participation in such activities, including, but not limited to, falls, contact with other participants, weather effects, such as heat and/or humidity, and conditions of the training facilities and services, and that you accept and understand all risks involved. After having read this resignation, and knowing these facts, and considering the acceptance thereof, by means of this document you resign and release BOZALE BUSINESS S.L.U. and their representatives from all responsibilities of assets and liabilities of any nature derived from their participation in any sports practice (including water activities) during the Coexistences.

Responsibility/obligations:

We only act as agent for the providers of the services mentioned in the itinerary or otherwise provide the services or goods in connection with the WOSPAC Activity, such as accommodation providers, field operators, bus lines, rental companies car dealers, driving services, restaurants and entertainment providers. We do not assume any responsibility for any personal injury, property damage or other losses, accidents, inconvenience or irregularities that may be caused by any act or omission of any of the Suppliers, any matter beyond our sole control. We do not have any special knowledge regarding the financial situation of the Suppliers, lack of security, dangers to health or weather risks. You are responsible for respecting authority and following the instructions of the Coexistence guide. You may be excluded from participating in all or any part of the Coexistences if, in our opinion, your condition or behavior is deemed unsuitable for the Coexistences or unsuitable to continue once the Coexistences have commenced. Ineptitude may include, without limitation, any behavior that, regardless of its cause, is inappropriate or offensive or interferes with the delivery of Cohabitations services, or may constitute a danger or embarrassment. In such event, our liability is strictly limited to a refund of the cost of repayment of any unused portion of the Units. If you choose not to participate in certain parts of the Coexistence or use certain assets included in the Coexistence, no refund will be made for the parts or property not used in the Coexistences. The client will be responsible for any cost, damage or loss arising from its actions or omissions.

General:

Any conflict related to the Coexistences will be governed by Spanish law, without taking into account conflicts of legal principles. The courts of Barcelona, Spain will be the exclusive forum for any claim related to the Activity, and you consent to the personal jurisdiction of such courts. You agree to file any claim against us within 30 days after the Cohabitation ends and to file the claim within one year of the incident, and you acknowledge that this is expressly limited to the applicable one-year statute of limitations. . You agree to grant us (and third parties with whom we may co-market) a fully paid, royalty-free, worldwide license to use your photos, videos, or digital images solely for our promotional and/or commercial purposes. . The transmission by facsimile of any signed document will be considered as the delivery of an original. If requested by us, you must promptly submit an original document as well. If there is any conflict between this Participant Agreement, the booking form and the itinerary, this Participant Agreement will apply.

Release of Responsibilities:

I, as a parent or guardian, give my consent to provide my child with the prescribed and/or over-the-counter medication(s) listed below during summer camp, always adhering to the dosage and schedule listed below.

IBUPROFEN and PARACETAMOL

The parent/guardian must deliver the medication(s) to authorized program personnel in a sealed plastic bag with sufficient instructions regarding dosage and schedule of administration, as well as any other pertinent information. Camp staff will not be responsible for administering any medications that require nursing training or special skills, such as diabetic or allergy injections. It is mandatory to inform the admissions staff about any special medical needs to determine if the youth can be accepted in the program or not. Children with contagious medical conditions will not be allowed to attend camp.

I/We, as parent(s) or guardian(s) give my/our consent for our child, identified herein, to participate in swimming, sports and digital activities. I/We understand that participation in any swimming activity is at our own risk and expense. I will not/will not consider BOZALE BUSINESS S.L.U. its employees, agents, representatives or anyone acting on its behalf, as responsible or guilty of any damage that may occur to the named minor during said activities or transportation.

I/We understand that BOZALE BUSINESS S.L.U., will complete the necessary accident insurance forms, after which all claims under the policy or insurance policy for injuries suffered by the participants during the activities and events carried out in the Coexistences, will be processed by the minor, their parents or guardians through the company that manages the minor’s insurance policy and not through BOZALE BUSINESS S.L.U.

I/We assume financial responsibility in case of loss of personal belongings by the minor in question.

I/We authorize BOZALE BUSINESS S.L.U. to transport and obtain, through a physician of her choosing, any medical care that may be reasonably necessary for the minor during the course of such activities/events or such travel. I/We accept that the expenses of said trips and treatments will not be the responsibility of BOZALE BUSINESS S.L.U. I/We accept full responsibility towards them and, by means of this document, I grant permission for my son/daughter to travel on any trip related to the Cohabitations, whether by bus, minibus, van, or car.

Weapons: Students shall not carry or possess any firearm, knife, weapon, or object that could be used as a weapon. It is made known that the possession of a firearm, a knife, a weapon or an object that can be used as a weapon, is grounds for immediate expulsion from the Convivencias. Parents or guardians will be notified immediately in order to pick up the minor the same day, and WOSPAC will not be responsible for the minor from the moment of communication with the parents or guardians. The amount paid for the Coexistences will not be refunded.

Illegal substances: It is made known that the possession or sale of controlled substances, by any person, will be grounds for expulsion from the Coexistences. Possession of or being under the influence of alcoholic beverages and/or hallucinogenic drugs or a combination of drugs or substances that have hallucinogenic effects, marijuana, glue, or other drugs or combinations of drugs or drug paraphernalia expressly prohibited by federal law, state or local; including prohibited substances that may include said substances possessed, sold and/or used that are considered or may be considered as controlled or illegal substances, or controlled and/or illegal substances modified in any way will lead to the expulsion of the rest of the Coexistences and of the minor’s information to the corresponding security agencies. Parents or guardians will be notified immediately in order to pick up the minor the same day, and WOSPAC will not be responsible for the minor from the moment of communication with the parents or guardians. The amount paid for the Coexistences will not be refunded.
